Each will have one of the three trainers to work with, those three which includes the return of Jillian Michaels. Alison then calls out the names of the fifteen people who will be this season's contestants and who have no idea they have been chosen until their name is called. Each contestant gives a little background of him/herself. Once at the ranch the next day, five adult contestants apiece are assigned to one of the three trainers, Bob, Dolvett and Jillian, but realistically any contestant can call upon any one of the trainers for any advice or assistance. The fifteen plus the three youth are thrown into the fire with their first workout. Jillian in particular has issue with three of her team members, and tries to find out why they have what she considers a defeatist attitude. The first team challenge of the season, which includes the three youth, is to find letters in a ball pit to spell out the phrase "The Biggest Loser", with the winning team getting a five pound advantage at the weigh-in. The three youth leave the ranch for home before the last chance workout, which takes its emotional toll. At the first elimination weigh-in, the contestants learn that there will be no yellow line, but only a red line, meaning that the person with the lowest percentage of weight loss on the losing team will automatically be the first eliminated contestant. However, something that happened at the last chance workout may increase that number by one.

All thirteen remaining contestants learn that at this week's weigh-in, there once again will only be a red line, meaning that the person with the lowest percentage of weight loss on the losing team is automatically eliminated. For their first workout of the week, they will be doing it in an outdoor workout space specially designed by their trainer. Dr. Huizenga provides a medical assessment of all contestants, and the news he provides is generally not good. The three childhood obesity ambassadors return to the ranch to have their own physical assessment done with their trainer. They join their adult counterparts for the week's team challenge, which is modeled on football as their guest is San Diego Charger Antonio Gates. Although it is high on their minds during the entire week, the thirteen contestants are focused on the dreaded week two minimal weight loss curse most specifically during the last chance workout and at the weigh-in.

Alison gives the teams a trivia test on childhood obesity, the losing team which is given what they consider the worst punishment: having to spend four and a half hours every day this week locked in a typical childhood obesity room filled with junk food, televisions and video games. As that time takes away from their workout time, the trainer of the eventual losing team is determined to make the best out of this bad situation. Two contestants on two different teams have a bad week mentally, while another decides at least temporarily to switch trainers to get a different perspective both on workouts and the emotional cause behind her weight gain. The three childhood obesity participants, at home this week, are visited by "Dr. Joanna" who gives them and their families health and nutrition advice. The week's team challenge is at a make-shift carnival, which are notorious for junk food. The contestants literally are weighed down by this junk food in this challenge. At the elimination weigh-in, the contestants learn that there is no red line this week, but will be the first time that the losing team will vote out one of their own, the only person on that team immune being the one with the highest percentage of weight loss. Alison shows them this season's new voting room, which none want to have to visit again.
