- Some more intense kissing scenes, but never any nudity. Some scenes that imply the characters will engage in sexual activity straight afterwards, but that itself is not shown.
Violence & Gore

- Several surgeries are performed, but the work itself is always 'off-camera'. Sometimes some blood stains can be seen on Michaela's dress and hands. Patients are always put under an anesthetic in such procedures, except for in season 6, when Michaela needs to burn out a wound in someone's leg. Some screaming and struggling is involved there.
- In several occasions, broken legs, ankles and shoulders need to be set in place. Rarely can the bone be seen penetrating the skin. The victims usually give a firm scream.
Profanity

- No other category fits except that the N-word is sometimes used so this fits. Along with that word, race is often a topic where former slaves are put down solely due to their race. There is an overarching attempt to show how the behavior is wrong.
- "Damn" & "Hell" used in most episodes at least once each episode.
- Because of the continuous occurrence of the native American Cheyenne tribe, some characters will refer to them with unkind words and nicknames. The same happens upon the introduction of Chinese, Hispanic and other ethnic 'minorities'.

- This shows rating is ridiculously low for how graphic some of the stuff is, including a man getting hung for r@ping a very young girl
- In the first season, a man appears with significant burn scarring on his face (ep.10). It can be a rather shocking sight.
