- There's a scene where a group of men in a night club admire a prostitute. We hear yelling and whistling of them.
- Graphic full frontal nudity is shown many times throughout the film.
- There is nudity in the scene where a nude woman takes a bath. Only her head, body and legs are seen.
- There is a protracted rape scene which is very graphic. The woman's breasts are visible, as well as thrusting. Very uncomfortable, long, disturbing, and graphic scene.
- An attempted rape scene occurs (different form the earlier rape scene). The woman's clothes are torn off. Her breasts are seen for a long time, but her genitalia is partially obscured. Very disturbing and graphic.
- The main character is shown nude many times throughout the film. Her breasts, butt, genitals, and pubic hair are all clearly seen many times.
- Lots of graphic full frontal nudity throughout, as well as extreme close ups of the main character's breasts, butt, and hairy genitals.
- There is a graphic rape scene with breast nudity. Very disturbing.
- A woman gets naked while the photographer takes pictures of her. Some animated nudity.

- A man punches another man in the face during Cham's concert at the beginning of the movie.
- A man has been stabbed many, many times. He gets stabbed in the eye, the hand, the stomach, the crotch and more.
- A man is brutally killed, with his eyes poked out, while his phone rings.
- A woman gets stabbed in the stomach with glass.
- There's an act going on and the Powertrons use their guns at Kingberg.
- There's violence of a manager hitting some guys at a strip club and he falls down and a woman falls while being grabbed by the ankles, for a show.

Frightening & Intense Scenes

- Film portrays a very severe form of psychosis that may be triggering for some.
- Although this film is disturbing at times, there is nothing that explicit or graphic, the rape scene (while disturbing) is not explicitly shown. The violence is more intense than graphic.
- Perfect Blue is a very disturbing and intense movie. There is a simulated rape that will still be found disturbing by some. The aftermaths of the murders will frighten people, and the one shown murder is very intense.
- The ending will definitely qualify as frightening, disturbing, and very intense as well.

Sex & Nudity
- A man has a woman pinned down as he rips off her clothes with intention to rape her. She is able to fend him off before he does anything.
- When a man is stabbed to death, it is intercut with brief and graphic shots of a naked woman.
Violence & Gore
- Rumi bends over a broken window to get Mima's wig, and in doing so is impaled on some glass shards. A lot of blood flows.
- Mima gets stabbed by an umbrella and an ice pick.
- Me-Mania is knocked unconscious by Mima and later found to be killed along with Tadokoro. Their murders are not shown, but their bloody dead bodies are seen together.
Frightening & Intense Scenes
- The most intense scenes are mostly found in the second half of the movie.
