- Strong implications of sexual intercourse and rape in the Caves as loud moaning and panting can be heard, suggesting an orgy is taking place.
- In the Butchers lair and later in the last four levels of the game in Hell there're various dead naked bodies that have been hacked apart and tortured. They aren't very detailed, but breasts, nipples, and pubic hair can be seen. Also later in the game you fight Succubi who all have breasts and nipples visible. In the final cutscene at the end of the game you can briefly see Albrecht's dead naked body. Genitals are visible, but obscured by shadows.

- Bloodied corpses are seen frequently throughout, some appear bisected with their limbs missing while others are chained, strung up or impaled on spikes. There are pools of blood with viscera and organs strewn around, indicating a person has been devoured by the demonic inhabitants.
