Sex & Nudity

  • In one scene you see a man in just his underwear *tighty whiteys* staring at a wall lasts maybe a minute or less
  • In another scene the same man you see in his underwear is seen lying on the floor naked covered by a clear sheet *nothing shown besides chest*
  • A photo of a nude mental patient is briefly visible on-screen; her breasts are visible but partially covered by her hands. A newspaper chipping photo of naked human bodies in open graves is also shown briefly. Neither photo is intended to be sexual.
  • A man is briefly seen showering (naked from the waist up; the scene is blurry and very short).
  • Hank repeatedly teases another character after sleeping with his ex-girlfriend, Amy. He states that he "only fucks her to piss off Phil".
  • Pranksters have drawn graffiti of a penis and breasts on the wall of the old hospital.
  • It is discussed that one of the former patients in the hospital had a (non-consensual, for the patient) incestuous relationship with her father and grandfather.

Violence & Gore

  • A character cries out in pain; his leg is revealed to be severely burnt. Blisters and dried blood are briefly visible.
  • It is mentioned at various points that a man has hit his wife.
  • A 1950's murder is discussed, in which a young girl had a psychotic breakdown and was convinced by a sinister voice in her head to "cut up" her brother and parents. This is never shown onscreen, but as the story is being told, various screaming noises and stabbing sound effects are heard.
  • A vintage photo is briefly seen of several deceased human bodies lying in open graves.
  • Blood is seen dried onto roses, onto photos of an infant's christening, and on an antique coin.
  • A man coughs violently; it is revealed that he has breathed in a massive quantity of asbestos dust.
  • Descriptions are made of some archaic treatments from a mental hospital, including electroshock therapy, hydrotherapy, art therapy and the pre-frontal lobotomy. Most of this is only verbal dialogue, however it is often quite graphic.
  • Several bloody bodies are shown, and you get to see flashbacks of the murders. Many of them are moved, leaving a large bloody smear on the floor/tarp.

Profanity

  • Some sexual terminology and innuendo: "bang/banging", "orgy", "virgin", "ass", "abortion", "rape" and "pregnant".
  • In a bizarre scene at one point, the film directly zooms in on a man as he declares in a lengthy, drawn-out way, "fuck you"; this disturbs another man.

Alcohol, Drugs & Smoking

  • A main character sneaks up to a rooftop by morning to roll a joint; marijuana buds on paper being licked into a rolled shape is briefly seen, after which the character smokes it.
  • A character is told not to do any drugs while working.
  • A brief cutaway scene shows a man sitting alone in a bar, surrounded by people talking and drinking alcohol.
  • The characters are seen smoking cigarettes throughout the film.

Frightening & Intense Scenes

  • The entire film has a very grim tone.
  • A man listens to taped psychological sessions with a female mental health patient several times. The patient is suffering from Disassociative Identity Disorder (formerly Multiple Personality Disorder) and her personalities speak in various ways that may be scary. One of her personalities also tells a frightening story which includes murders.
  • Characters losing their mind during the movie may be frightening.
  • Several scenes shot in a dark cellar creates a scary mood. One of the cellar scenes shows a man running and screaming through the cellar trying to get out before the lights go out.
  • Blood stained pictures of a young baby are shown.

Spoilers

The Parents Guide items below may give away important plot points.

Violence & Gore

  • A man is shown with an antique orbitoclast (the medical term for the 'icepick' type lobotomy tool referred to frequently in the movie) embedded in the inner corner of his in his eye, all the way to the hilt. His eye socket is bruised, there is some blood, and his eye is very bloodshot. He turns out to still be alive, and the lobotomy tool is sloowly pulled out (graphic).
  • The pick is then used to kill another man. In a flashback we see the murderer insert the ice pick-like tool into the victim's eye socket, while the victim twitches.

Frightening & Intense Scenes

  • A character has a flashback to when he killed his wife and baby. You don't see anything as the murders occur off-screen after the character has parked his car and entered his house, but you hear the screams of his wife and hear the chaos (intercut with some of the blood-stained photos referred to above) which may be disturbing to viewers.
