Sex & Nudity

  • The chao creatures mate with each other. The process, though, is not explicit.
  • Amy Rose wears a short dress, and her underwear is often visible as a result.
  • Rouge's cartoon illustrations show some cleavage, though her in-game model does not. There is an unlockable outfit for her however that is very revealing. The song "Kick the Rock!" (played during one of Knuckles' levels) contains some suggestive lyrics about Rouge, calling her "sexy and smooth".

Violence & Gore

  • Most in-game action is standard Sonic fare, smashing and shooting robots in fairly cartoony ways. Several cutscenes up this ratio though, including having Dr. Eggman aiming a gun at Amy, then he seemingly kills Sonic by launching him into space in an exploding capsule (though he escapes). Two characters are gunned down offscreen, one of them by a firing squad. The final boss of the game, a living creature, is clearly killed in battle and hangs off a space colony lifelessly. Throughout most of the game, the entire planet is in danger of being destroyed by a space colony.

Profanity

  • In the first cutscene, before the player enters City Escape, the man on the radio says, "What the hell?" in the demo version, but it is replaced with "What?!" in the full game.
  • On Steam, there is an achievement called "heaven or hell", though it is referencing the literal place and not intending to be profane.
  • The song "Death Chamber" (played during one of Knuckles' levels) and Knuckles' theme song contain the word "damn", though it is not present in the game and is only featured in the soundtrack. No characters use profanity, though there is some mild insults and name calling.

Frightening & Intense Scenes

  • The music before sonic drowns is very stressful and maybe even considered scary by some players.
  • In certain levels, there are ghost enemies that are very creepy looking in appearance. There is also a giant ghost boss in one of Knuckles' levels, which may disturb younger players.
  • The whole plot of this game is very intense, and caution may need to be taken for younger players.
  • A 12-14 year old girl gets shot offscreen, and she lies on the ground while she says her last words. This may be intense for some players.
  • At several points, Tails loses his temper (specifically at beginning and then when Dr. Eggman seemingly kills Sonic), which might frighten younger players.
  • This game has a much more serious tone and story than previous Sonic games.
  • Dr. Gerald Robotnik's journal is very dark and talks about death. It can be unsettling or even frightening for players as it portrays him descending into homicidal insanity more or less realistically. He is implied to be killed by a firing squad at the end of the journal.
  • The true final boss of the game is a giant genetically engineered monster that resembles a lizard/dragon with weird proportions and cyborg implants. This may frighten younger viewers.

Frightening & Intense Scenes

  • After the Final boss, Shadow falls to the Earth and is thought to have died, with all the characters paying their respects to him during the credits scene. Though it's revealed in the next game "Sonic Heroes" that Shadow survived the fall and is placed in a Cryosleep-like state. However, this can still be upsetting and intense for some players.
  • The moon is blown in half. Civilians watching this start screaming in panic. A countdown clock for 24 hours is then shown, stating how long until the earth is also blown up, unless the world's governments surrender.
  • In the Japanese version of the game, Maria, A 12-14 year old girl, is dying from a terminal illness called N. I. D. S, (based on the real-life A. I. D. S virus). While this was never mentioned in the US version of the game, it is still in the game's manual.
