In the early episodes, Hot Shot is sometimes called Hot Rod. Later when Tidal Wave appears, he is sometimes called Shockwave. Mixing up the character's English and Japanese names was a fairly common mistake in the dub. Even Optimus Prime himself was occasionally referred to by his Japanese name, Convoy, and at one point even Optimus himself mistakenly used that name. Overload is also called by both his English and his Japanese name (Ultra Magnus) at different points.
The names of the Mini-Cons constantly get mixed up in several episodes. Leader-1 is the name of Megatron's Mini-Con, but at instances Optimus Prime's Mini-Con (Sparkplug) is referred to by that name. Sometimes the names of Grindor, Hard-Wire, and Sureshock get mixed up as well.
Several times throughout the series, Starscream is drawn with both of his wings even when he's wielding his sword. This would be impossible, since his sword is actually his left wing.
Occasionally, the blond-haired Rad is drawn with a darker-colored hair, and in some shots, the Latin-American Carlos is colored like a Caucasian.
Sideways' "standard" head is erroneously colored grey in the few scenes it appears in, even though it's meant to be purple and yellow like his body. It is colored correctly only during his stock footage transformation scenes, during which this head is briefly visible.
Due to the pressure of the American dubbing studio, the animation had to be rushed, and this resulted in numerous animation errors. These include characters growing huge or shrinking to a small size between shots, the absence of lip-movement when someone is talking, and a constant shuffling of color schemes. Sometimes, even the background art was inconsistent. Another animation fault of the early episodes was sliding still pictures over the screen instead of animating the characters fully.
English version: In many episodes, the voices of the characters change randomly. This is most blatant in episode 50 (Union), in which Demolishor spends the entire episode (apart from a single line) talking in Hoist's voice.
Overload, the giant red Transformers who can combine with Optimus Prime simply appears among the Autobot crew from one episode to another, but it is never explained where he came from or who he even is.
English version: the Grand Canyon is erroneously called "Big Canyon".
English version: Lightyears are often used as a unit of time measurement, when in fact they are units of distance.
English version: When leaving Cybertron, Optimus entrusts the planet's protection on a shadowy figure who he calls Thrust. It's actually Jetfire - Thrust is an evil Decepticon, not one of Optimus' men.
