- Ada walks around with a short skirt, showing off her thigh region.
- The Meta Quest 2 VR port removes all suggestive comments and dialogue from cutscenes and gameplay.
- There is a brief scene where Luis Sera comments on Ashley's breasts. She considers this to be rude and irrelevant.
- If Ashley is on a ledge above Leon and in her default attire, the player can (un)intentionally look up her skirt if aiming upward at her. Ashley will respond by shrieking, holding her skirt down. and calling Leon a "pervert".
- At the end, Ashley asks Leon if, upon returning her to her dorn, they could put in some "overtime" (meaning her sleeping with him as a token of gratitude), but Leon politely declines.
Violence & Gore

- Several enemy types are capable of decapitating the player. The death animation has blood rapidly spewing from the player character's neck as their head gets cut off and falls down to the ground. Censored in Japanese release
- The Japanese release of the game has some of the more graphic violence censored.
- When Leon is choked, his neck snaps and spews blood.
- One of the goriest games in the series besides its remake and resident evil 7
- Most enemy types can be decapitated via headshots from firearms. A loud gory noise is heard and the enemy character's head explodes into chunks. Both the headless corpse and the chunks despawn quickly due to technical limitations. Censored in Japanese release.
- Certain enemy types later in the game have an ability to remove their head to reveal an mutated parasite underneath it. Rather graphic.
- Stronger weapons like shotguns or sniper rifles have a higher chance of decapitating enemies than other weapons. Censored in Japanese release.
- Brutally mangled and blood soaked corpses can be found within the game environment.
- Players fight with a wide array of weapons (not that much of a factor, though).
- Explosive weapons (such as Hand Grenades, Rocket Launchers and Mine Throwers) will cause enemies to fly though the air. They also have a random chance of complete vaporizing the enemy into splash of blood. Rather graphic.
- Incendiary Grenades (Fire grenades) will cause enemies to burn.
- Aside from the numerous enemies to kill, the player is also rewarded with cash for shooting animals (specifically crows, chicken and fish) that occupy the environment.
- Rather gory and gruesome scenes of humans mutating into monsters. Graphic
Profanity

- The enemies use strong language in Spanish, such as "cobron" (motherfucker) and "puta" bitch.
- The word "bitch" is used at least once by Krauser. When Krauser and Leon are fighting the first time, Ada intervenes and stops the fight. Krauser then says" Well, if it isn't the bitch in the red dress"
- Leon uses a religious profanity "In the name of God!"
- The game includes bilingual swearing (English and Spanish). However, no Spanish dialogue is translated.
- "Shit" and "Hell" are said by Leon a few times throughout the game.
Alcohol, Drugs & Smoking

- One of the Spanish cops offers Leon a cigarette. He declines.
- Luis asks Leon for a "smoke", but is out of luck; Leon only has gum.
Frightening & Intense Scenes

- The game has an atmosphere of tension and dread, with frightening images and sounds. The soundtrack is also very chilling.
- It is a story-based adventure with horror themes and by today's standards is rather tame
- All of the enemies in the game are scary and deadly. They become more and more frightening as the game progresses, including insects, regenerators, giants and countless other monsters. The scariest ones however, are the Regenerators.
- The scariest part of the game is when you have to fight these creatures called "Regenerators". They're very tall, slender, frozen, red-eyed, gray-skinned, sharp-toothed, monstrous looking beings that make scary noises, are very difficult to kill since they have the hability to regenerate (hence their name) and are extremely agressive, vicious and unrelenting. They're known for being some of the scariest video game creatures.
