- In one of the first scenes in the game is in a sperm donation clinic, where you go into a room to "collect donations". In this room, there are images of real half-nude women plastered on the walls.
- Cover has a woman with a see-through top
Violence & Gore

- As in the first game, your player can beat up, stab and shoot people, often with bloody results. However in this expansion pack, weapons such as a machete and syth are added, which enables players to dismember humans.
Profanity

- Profanity levels are similar to the second game, with words like "fuck" being used.
Alcohol, Drugs & Smoking

- Players can smoke crack cocaine which boosts their health. Players also become addicted to it and lose health when they do not smoke for a while.
Frightening & Intense Scenes

- There are some segments in the game where the visuals change to simulate hallucinations caused by your character's head wound. In these segments, the walls change to a "fleshy" texture and the doors become an edited picture of Gary Coleman's character with blacked out eyes. Mildly disturbing. May cause seizures due to the textures moving erratically at points.
- This expansion pack features zombies, the first level in the hospital is fairly disturbing and scary.
