When Carley gets into the limousine at the end, she enters on the left side back seat door. In a close-up seconds later, she is shown on the right side of the car in the back seat. Then, as the limousine pulls away, she is back on the left side of the car.
Update: The references to right and left side of the car are made from the point of looking at the car from the front. In actuality, when sitting in the car, Carley more correctly starts by sitting on the right (passenger's) side of the car as she is talking on the phone, then appears to be on the left (driver's) side of the car in the close-up when she hangs the phone up, then is back sitting on the right (passenger's) side of the car when they drive by Banacek and the nurses.

When Banacek runs back after closing the electric fence to electrocute Chris Bailey, the fence is open. That's because when the high voltage is cut, either by security card or (in Banacek's case) throwing the circuit breaker, the gate swings open automatically, as is seen.
In the introduction, as MAX is being switched on and off, the close-ups show the two power switches being flipped up (toward the display console) to turn MAX on, and down to turn MAX off. When Leslie Lyle requests a preview demonstration of MAX, Logan flips the switches down to turn MAX on.
In the end, when Carley appears to switch places in Banacek's limousine (see the continuity entry), the switch actually occurs due to flipping the negative. When Carley is talking on the phone standing at the rear passenger's side of the car, the buttons on her jacket are on her left side (the right side of the jacket has the buttonholes and overlaps the left side). Then, in the close-up, when she appears to be on the driver's side of the car and hangs the phone up, the buttons on her jacket are on her right side, and the left side of the jacket has the buttonholes, overlapping the right side of the jacket. Furthermore, during the introduction, when Jay answers the initial call from Cavanaugh, the phone is located on the passenger's side of the car. When Carley hangs the phone up at the end, the phone appears to be located on the driver's side of the car - verifying that the close-up section of the film was flipped during the process of putting the final print together.
Obvious stunt double for Banacek at the start of the fight with Chris Bailey. George Peppard ducked off camera at the last second, then the camera pans to the stunt double without any cuts in-between.
