Sex & Nudity

  • Brucie discusses "bull-shark testosterone" steroid injections, to which Niko makes a comment about it effecting "your balls."
  • An in-game cartoon shows 2 space marines go under covers and the covers are seen rumbling and the covers float away and then the marines chase the covers to cover themselves up. No nudity is shown and it's very cartoonish.
  • There are women implied to be hookers that walk around and are dressed in fairly provocative clothing.
  • One mission involves Niko going into a sex shop although you cannot interact with anything and all of the posters and videos are nearly impossible to make out due to poor resolution it just makes it look really grainy and blurry.

Violence & Gore

  • Fist fights using martial arts moves both free roam and cut scenes
  • Lot of fights using stylized punches and kicks
  • A man is smacked twice with a hacksaw in one cutscene.
  • One mission requires the player to kidnap a woman. On the way to the safehouse, she tries multiple times to distract the player before he punches her.
  • Lots of extreme bloody shootouts with guns.
  • Lot of action including fights with both hands and guns , car chase and violent shootouts.
  • You can kill inoccent people and cops also other criminals blood splatters when people are shot.
  • Lots of bloody brutal violence throughout.
  • The player can hit people from a car which results in minimal blood splats. You can shoot, hit or knife people which also results in a minimal blood splat. Pools of blood form around dead bodies.
  • People can be set on fire with molotov cocktails or as a result of explosions. This isn't very graphic but their charred corpse can be seen afterward.
  • In a cutscene, a man is shot through the eye. This is shown from the behind the man, but blood sprays out and his corpse can be seen later.
  • Two men are shot in the head, one of them through the eye. However there is little blood shown.
  • A man is shot in the chest. Blood appears as the man collapses and dies. This scene has an emotional aftermath. The gunman is then shot several times with a pistol and pump shotgun.
  • A man falls from the roof of a project to his death. He screams as the falls down.
  • At one point in the game the player must kill a man in hospital by switching off his life support system.
  • A man can be executed by being shot 12 times in the chest.
  • Other executions and cutscenes include men being shot with typical sprays of blood.

Profanity

  • One use of the N word although it is very easy to miss.
  • Fuck, shit, and bitch are used occasionally but not often while other milder terms such as Damn, and Hell are used more often.
  • Occasional uses of fuck and 1 use of whore.
  • Cinemax content advisory for footage of the game: rated TV-MA-LSV, graphic violence (GV), language (L), and drug use (DU)
  • In one cutscene Niko sees a scantily clad prostitute and says "Another common liberty city street whore" although that is the only use of the word in the game.
  • The language in this game is not as bad as in San Andreas, but there is still occasional use of strong language.
  • Additional foul language such as "Fuck", "Shit", "Bitch", "Damn", "Hell" and some limited uses of "Whore" are also used.
  • There are various uses of religious slurs such as "Goddamn" although that is not often said.

Alcohol, Drugs & Smoking

  • A man is implied to be a heroin addict. He is never shown using it but it is talked about.
  • A woman is implied to be a heroin addict. It is never shown but is talked about.
  • This game takes place around the same time that the smoking ban in New York City was first being implemented. One of the characters in-game ignores the law and smokes cigarettes in a bar and numerous characters make dialogue references to this.
  • One of the characters is seen pretty often smoking a marijuana joint and is said to be a dealer.
  • There is one mission where you must find a stolen bag of cocaine although the substance is never shown you see the bag with white powder in it.
  • Players can become intoxicated and drive. The game penalizes the player for doing so by making it difficult to steer and drive, and any nearby police will also converge on the player if this attempted. The option to hail a taxi is given, but due to inability to walk properly, it is made very difficult to actually do this, which may lead some players to simply try to drive.

Frightening & Intense Scenes

  • Grand Theft Auto IV is an open-world violent crime drama with lots of strong bloody violence, drug use, sex and numerous amounts of criminal acts.
  • Darker and more violent than GTA V but way tamer on cursing and sexual content.
  • Rated R for Strong Brutal Violence, Language, Sexuality and Drug Use.
  • Niko's past is discussed at length throughout the game's plot, particularly him being involved in the Yugoslav Wars and a traitor giving his squad up for money. On some occasions he describes the events of the war, saying he "watched my friends die", and saw "a line of fifty children with their throats slit and their hands chopped off", and that "it ruined me".
  • The dramatic nature of the game, although layered with comical elements, may unsettle some players especially with the extreme amounts of violence. The story's depth is surprising and emotional.
  • Many viewers may consider the violence, anti-law, sexual content, drug use and language to be offensive.

Sex & Nudity

  • Niko is implied to have had sex with some female characters in the game.

Violence & Gore

  • Another man is shot in the head. A man is then shot in the stomach, however he survives. Some blood shown.

Frightening & Intense Scenes

  • Towards the end, Niko finally encounters the traitor that gave up his squad for money, Darko Brevic. Darko is seen drugged, insane, and emotionally unstable, and he begs Niko to kill him. The player is given the option to kill Darko or let him go free. Either case involves an emotional aftermath.
  • Towards the end of the game, one of two possible characters, Roman or Kate, is killed in either a drive-by shooting or a stray bullet from a failed assassination attempt on Niko. Both scenarios are emotionally upsetting in their own right.
  • Roman's apartment and cab building are burned down in arson attacks. He cries and screams angrily, and explains that he left a ring in the apartment with which he planned to propose to Mallorie.
