Sex & Nudity

  • One of the characters, Paz, is implied to have been sexually assaulted when she was imprisoned at a facility.
  • The same mission, should the character succeed in getting an S rank, will also have Paz in her undergarments.
  • Similarily, an optional mission involving dating Kaz (a male) can be unlocked, with a similar ending and similar unlockables.
  • A character is implied to be bisexual.
  • Kazuhira Miller is implied to be the byproduct of prostitution after the events of World War II.

Violence & Gore

  • A man punches another man.

Profanity

  • 1 Bullshit, several hells, damns, bitches (including son of a bitch), bastards, at least one use of the lord's name in vain, asses, and at least one use of "frickin'" (IE, "Time to mince some frickin' meat!"). One use of piss.

Alcohol, Drugs & Smoking

  • The main character, Snake, often smokes Cigars.
  • Some of the characters in the game, Amanda's Sandinista National Liberation Front unit, were implied to have trafficked and purified drugs under the guise of a Banana Factory in order to gain the funds needed for their weapons, troops, and food.
  • In one of the briefing tapes, one of the characters, Cecile, is implied to be drunk from wine.

Frightening & Intense Scenes

  • In the beginning of the game, Snake sneaks into the storage facility, and while sneaking, he sees a bloodied corpse of a person, implied to have been beaten to death by the Peace Sentinels to extract intel about his troops. This scene might disturb some viewers.

Sex & Nudity

  • Same with Kaz.
  • In an unlockable and optional mission, Snake (or another character) can go on a date with Paz, and the mission's completion implies that they were making out.

Violence & Gore

  • At one point, Hot Coldman shoved a man in a wheelchair (Huey Emmerich) down the stairs.
  • Snake is shocked by a stun rod, and it's implied that he received a dose of at most 1.5 million volts.
  • In a dream flashback, Snake and a woman (The Boss) are fighting in the middle of a flowery battlefield. This is a reference to Metal Gear Solid 3.
  • A man is shot in the chest twice by a Soviet KGB agent.
  • A woman uses laughing rods (only in the Japanese version), implied to be lightning rods, when torturing a man.

Alcohol, Drugs & Smoking

  • Several other characters are also implied to smoke, including one character who was underage. (Although, Paz is later reveled to be 21.)
  • A woman, Strangelove, has Snake use a substance called Snuff (implied to be dried Tobacco leaves ground up) as a substitute to Cigars when entering the AI facility
  • Paz, when revealing her true self, spits out snuff tobacco.

Frightening & Intense Scenes

  • The scene where Coldman explains that Peace Walker is completed and his first target may be a bit disturbing to some viewers
  • The final moments of Chapter 4 (the ending of the Head to the Control Tower mission to the end of the second Peace Walker battle) is very intense, as a KGB Agent betrays Coldman to launch a nuke at Cuba, and intended to kill Snake. Also, Coldman, in his dying breaths, had activated Peace Walker which wasn't only programmed to launch a nuke at Cuba due to false trajectory data, but also transmitted the same False Data to NORAD, which nearly caused an all out nuclear war.
