Sex & Nudity

  • In the beginning you follow Ishmael, who is wearing a hospital gown. Glimpses of his butt are seen through the back of the gown.
  • Quiet and the female skulls have overexaggerated jiggle physics applied to their breasts. While there is no nudity, the camera sometimes lingers on female characters' breasts and buttocks during a few cutscenes. These are all skippable however.
  • There are a few ways to trigger a cutscene where Quiet and Snake take a shower together. The camera is in first person and Quiet is still in her bikini, however Quiet is in focus during this cutscene. And the male characters around her cell watch her as she showers.
  • The character Quiet wears nothing but a bikini top, bikini bottom and pantyhose that reveal and accentuate her breasts and buttocks. In one scene, she strips out of her pantyhose and rolls around in the rain - becoming slick and wet. In another scene, if Venom Snake doesn't shower for a long time, Quiet will take a shower with him (albeit clothed in her bikini).
  • Some female characters show cleavage.

Violence & Gore

  • A woman violently stomps on a man's testicles, squirting blood everywhere.
  • Definitely one of the most violent games in the series.
  • How violent the combat is in this game depends on how far you are from the enemy. In close quarters combat, there is a big splash of blood (slightly more violent than Tomb Raider (2013).). But when sniping or from a distance, there is no blood (or the spray is so small or far away that you can't see it).
  • There is not much over-the-top violence during actual gameplay; there is a small blood splash when Snake shoots an enemy, and that's pretty much it. Violent cutscenes are few and far between, but they are very gory. However, all cutscenes are skippable.
  • The game rewards you if you choose to go about a mission with nonviolent methods. However it's important to note that killing enemies often makes missions far easier to complete.
  • In the course of the game, if the player kills very many enemies and/or does enough other villainous actions (such as killing animals in the warzone, shooting down friendly helicopters, ordering the development of nuclear weapons in Mother Base), Snake's shrapnel horn grows longer and his body becomes covered in dark red blood that can only be removed by doing things that raise his heroism point system.
  • The Metal Gear games have not been known to be containing excessive amounts of violence.
  • While there's only blood spatter in the actual combat portion of the game, a few cutscenes are extremely gory and violent.
  • The main villain Skull Face, in particular, is somewhat disturbing, as his skin was burned at a young age and thus has a skeletal visage. However, he appears rather infrequently and often wears a mask and a hat to cover his face.
  • Whenever Snake kills people, there's a small amount of blood on their clothes and a small amount of blood splatter.
  • Some missions tell the player to assassinate various individuals, although you are never forced to kill anyone and you are rewarded if you complete a mission without killing any enemies.
  • The player can use various methods to kill people in tanks, running over people, air strikes and shooting people with various guns.
  • In one cutscene, Quiet forces a knife into a soldier's mouth in an attempt to kill him. The man is shown with blood on his mouth as he attempts to hold back the knife. Eventually the knife breaks through his teeth, although he is saved before she kills him.
  • In a location called "The Devil's House", several people an locked away in a blood-splattered building.

Profanity

  • "F*ck" is said twice. One is quite hard to hear.
  • Some minor swear words such as "hell", "ass", "damn", "b*tch".
  • Some uses of "sh**", but this is rather infrequent.

Alcohol, Drugs & Smoking

  • Venom Snake routinely smokes cigars made from a "medicinal substance", wormwood (a real life medicinal plant). The player may use these cigars whenever they please to speed up the course of time during the day or night.
  • On the player's birthday, a humorous scene plays where Snake is given a cigar for his surprise birthday party. But he is never allowed to smoke it. If players have Quiet at this point, she will light a cigar for Snake by shooting it.

Frightening & Intense Scenes

  • Metal Gear has always been philosophical and does not shy away from dark deep themes. The themes in this game is child soldier, war, lack of "real" choices in life, America's influence on everything, rape, sex, identity, regret, and leadership.
  • If Metal Gear Solid V The Phantom Pain was a movie it would probably be classified as Rated R for violence, some grisly images, language, some sexual references, and drug content.
  • There is only a blood splash when Snake shoots an enemy. However, if you wound the enemy soldider enough, he will scream and then writhe on the ground in pain. The soldiers' realistic reactions to being shot can be disturbing.
  • The game is played in a way that where a lot of pressure is put on the player to not be caught by enemies. After some missions, players may need to take a break from the game to cool down.
  • The game starts with Snake waking up from a nine year coma, as he discovers the loss of his left arm and several other injuries. His reaction to these revelations are disturbing and realistic.
  • Innocent patients are shot down by soldiers in a hospital, which some people may find distressing.
  • There are very suspenseful and sometimes disturbing moments.
  • The game features child soldiers, and even sees the player fight them at certain points (with the objective being to avoid killing them). The realistic depiction of child soldiers is very disturbing.

Sex & Nudity

  • Quiet is nearly raped by a group of soldiers, sounds of pants unzipping can be heard, but this is interrupted before anything can happen. Everything is just implied, nothing is seen (besides Quiet's face).
  • In a flashback when Paz is being checked a second time for a bomb, it's implied they check her pubic area.

Violence & Gore

  • The most violet scene is quiets massacre scene. After nearly being raped, she attacks all the people who kidnapped her, she bites off someone's ear, and stomps a mans private areas off, and then stabs another many times, and then blows up a house with men in, then finishes the last one off with a sniper shot to the head. Afterwards, the player has to battle a lot of reinforcements. Very brutal.
  • Paz has a large scar on her abdomen. During one cutscene, Snake and Ocelot find that she had opened the wound herself.
  • Huey is tortured in several scenes, although never too violently.
  • The death of Skull Face is the most graphic scene in the game. His arm and leg are blown off with several shots from a shotgun as he screams in pain. His face, body and the ground itself are soaked with blood. He then begs for death before finally being shot in the head offscreen. This sequence is partly controlled by the player, who can choose to shoot him up to three times before a non-player driven sequence starts where Snake shoots him at least seven times regardless of if the player chose to fire or not.
  • Quiet is nearly raped by a group of soldiers, although she later escapes with the help of Snake.
  • At the end of the game, Snake (and thus the player) are forced to kill their own men to prevent a quarantine breach. The men are already infected and have no chance for survival, so the player is technically performing mercy killings. However, Snake's men beg him for mercy despite this. However, this is preventable, but is very hard to pull off.
  • Venom Snake mistakenly shoots a child and believes he has killed him.
  • Afterwards the same boy, having survived the injury, decides to commit suicide with a handgun before his island is carpet-bombed. However, a young Psycho Mantis saves this child: stopping him before committing suicide and elevating him to safety before the bombing commences.

Profanity

  • When the flaming man appears during "The Devil's House," Miller tells Snake over the radio to "Get the f*ck out of there."

Alcohol, Drugs & Smoking

  • The real Big Boss is seen lighting a cigar at the end of Mission 46.

Frightening & Intense Scenes

  • Eli, (Liquid Snake) is a leader of a Militia that uses children.
  • Skull Face, the Parasite Unit, the Man on Fire and Psycho Mantis may be very frightening to younger audiences.
  • An audio tape can be played in which Strangelove is slowly suffocating because she is trapped in an the Mammal Pod, and at the end she dies.
  • Skull Face's main goal is to use vocal-cord parasites to wipe out entire cultures based on the language they speak. His end result is to remove 80% of the world's population and control the remainder of humanity with nuclear power.
  • Quiet is almost raped by a group of soldiers before being rescued by Snake.
  • Towards the end of the game, Snake is bitten by a venomous serpent in the desert. As he lays dying, Quiet decides to radio help in order to save his life. After Snake wakes up, fully healed, he discovers Quiet is gone. It is revealed that by speaking over the radio to help Snake, Quiet triggered the fatal parasites in her own body. Knowing this will eventually kill her, she wanders into the desert wastelands to die so no one else will become infected with the parasites. The entire scene is very upsetting and emotionally intense.
  • At the end of the game, Snake is forced to kill his own men to prevent a quarantine breach. They beg for their lives. Some however, accept their fate and salute Snake as they die. Later on, all the bodies are cremated and Snake is shown tearfully spreading a soldier's ashes on his face, lamenting his actions. This is very heartbreaking and emotionally powerful, some may find it hard to watch.
  • The twist ending, that Venom Snake is not Big Boss, but merely a soldier that was used as a body double for the real Big Boss is very upsetting and intense. (also very VERY predictable) This is especially shown when Snake punches a mirror in a mixture of rage and grief.
