Sex & Nudity

  • A sexual assault is revealed to be staged on set for a film. It is only actors staring in a film that mirrors the first game. The assault itself is offscreen but heavily implied but as mentioned is revealed to be fake.
  • At one point, two presumed prostitutes can be seen with their breasts out. The game is pixelated, so it's hard to tell, but still noticeable.
  • When you select a level to play, they are presented as VHS tapes to choose from. One of these VHS tapes has a nude woman on it.
  • There is a level that takes place in a strip club, however you see nothing sexual inside of there and it looks just like a normal club. You wouldn't know it was one if one of the characters before the level said it was one
  • You can skip the sexual assault scene, when you skip it the character stands there instead of doing it.
  • In the opening stage, a man can be seen attacking a woman, getting on top of her while she's down, and pulling down his pants. This scene, however, is cut-short when a near-by director tells the man to stop (this is the reason HM2 was banned in Australia).
  • Few, mild references to sex.

Violence & Gore

  • Just like the first game, the violence is pixelated, but it's EXTREMELY graphic, gory and inordinate nonetheless.
  • Side Note: Hotline Miami is known for being extremely heavy on Blood & Gore, which is amped to the max in "Wrong Number".
  • Throughout the game, players are given the object to shoot, stab, and otherwise, attack enemies with several weapons. This causes a lot of Blood & Gore to splat out of the enemies, and, in the cases where you die, the player character.
  • Players are also given the option to execute knocked out enemies. Executions range from painting a floor with blood (slamming an enemies head into the floor hard, and repeatedly until death), kneeling down and shooting them in the head (it's not really shown at what part of the head the gun is pointed, though), finishing them off in a bloody fashion by beating them to death with a melee weapon, slitting their throats with a knife, cutting their heads/guts off/out with a chainsaw, etc.

Profanity

  • Strong language throughout, but not much other than a few F bombs here and there.

Alcohol, Drugs & Smoking

  • At one point, a presumed prostitute can be seen snorting cocaine.
  • One level contains multiple meth labs that the player can shoot and explode.
  • A mob boss recommends a retiring henchman some drugs he's producing.
  • Strong references to beer, drugs, etc. throughout.

Frightening & Intense Scenes

  • In addition to the brutal violence and difficulty, the game has a very depressing and somber tone throughout.
  • Hotline Miami 2 has extreme levels of violence that may disturb some players. Also, this game is much more violent and disturbing than the first game, as said before. The entire game is extremely gory and very brutally violent, and is undoubtedly one of the most violent video games ever created.
  • The fates of some of the characters are quite sad.
  • Throughout the game, several characters have visions that include a man in a rooster mask, who warns them about their future. This may disturb some players.

Violence & Gore

  • An actor dreams of being on a reality television show, where he's being interviewed. While being interviewed, the actor proclaims that he's always dreamed of murdering people (including kids), and hearing them scream.
  • An actor, while on set, is killed when a prop gun is confused with an actual gun.
  • Evan, a writer looking for information on a book, tries to enter a building full of mobsters to interview the owner, but, upon being punched, is forced to knock the man blocking the entrance out.
  • A retiring mobster henchman, while on drugs, is brutally beaten to death by three masked people.
  • Manny Pardo, a police officer who raids mob hideouts, killing several people, then claiming it as self defense, is sent to investigate several disgustingly graphic murders, followed by strange, cryptic messages like "I didn't mean to do it" written by the killer in the victim's blood. While playing as Manny Pardo, in specific parts throughout the game, you can open his trunk, in his trunk are some of the victims that are found dead. It's later revealed that Manny Pardo is the killer.
  • Two twins make it to the roof of the mob headquarters, only to find that their group isn't responding. Seconds later, they are both fatally shot in the head by the son of the mob boss. It's inferred that other group members may have been killed as well.
  • Manny Pardo, while investigating the mob headquarters, finds a man wearing a rubber mask resembling a cheetah. Insisting that he be arrested, the man in the mask is instead fatally shot by Manny, who claims it was self defense.
  • In one of the stages, a dead body is found near the entrance of a building, covered by a large square of cloth, and surrounded by blood.
  • A man is sent on a mission, which either ends in him dying, or being brought back to an building, where he is murdered by a man because he knew about secret operations he was running.
  • A soldier sent to raid a camp has some of his partners die, and is forced to rescue an injured friend. He later returns home, running a convenience store, but is killed by a nuclear bomb.
  • In the final scene of the game, a man visiting Hawaii with his mother hears over the news that the US and Russian presidents were both murdered in an attempted coup d'état. The Russians see this as an act of war and nuclear war begins. As the credits roll, all the characters are seen being killed by a nuke, even the main character from the first game.

Alcohol, Drugs & Smoking

  • A retiring mob henchman can be seeing getting high on drugs moments before being murdered.
  • A mob boss can be seen getting high on his own drugs, which leads to him murdering three masked men, and then jumping off of the roof of his own building, leading to his death.

Frightening & Intense Scenes

  • in the level ''Into The Pit'', if the player takes their time to look around the scenery, they will start to notice corpses being dissolved in acid baths, which can be disturbing to some players, although it is less noticeable with both the action and the pixelated style of the game
  • Towards the end it is revealed Manny Pardo (the detective) is the Miami Mutilator and he is scared that he will be found, there is a dream segment where he goes to one of the spots he killed one of his victims, there is a dummy sitting at a table with his gun and the dummy starts saying unsettling things with scary music in the background. The dummy chases you to give you a hug but Manny pushes him, this part feels like a flatout horror movie.
  • The Hawaii war levels are sad and intense, at the end one of your friends gets brutally blown in half by an elevator explosion having one member stay behind for them which leads you to rescue one member who turns out to be Jacket from the first game, after this Beard/The Soldier and Jacket form a bond but it is revealed Beard was killed in a bomb accident, it is very emotional and sad.
  • Hotline Miami 2 ends with the entire USA being nuked. This may be extremely sad to some players.
