"Ache" is an appropriate title for this episode, because that is what this was. This was one hour of Ernestine and Rosalee whining and crying and fighting humans and animals and bleeding. No substance, no story, just television that was trying to drive home the struggle both characters are facing, but instead accomplishing making the viewer really uncomfortable.
