Sex & Nudity

  • There are buttons players can click that say "try for baby" or "woohoo".
  • Sims can get pregnant for a few days until they eventually go into labor where you choose to deliver the baby at home or at the hospital. (Childbirth is not shown).
  • Sims can make love but it does not show it as it only happens offscreen. Such as under the covers in bed or in a closet.
  • Mods or cc where sims can wear revealing clothing and some where sims can be nude. It's never in the base game.

  • All Sims speak in Simlish, though the text boxes are sometimes suggestive.
  • When Sims take a shower or do other things, they get naked, though it is blurred out. Mods could filter out the blur.
  • Flirting and kissing can occur

Violence & Gore

  • Sims can get into fights (mild violence). Two sims are seen slapping eachother as a large cloud appears making it mostly invisible. Occasionaly during the fight they are seen slap fighting or having the other sim in a headlock. However these are barely violent at all and can be seen as comedic to some.
  • With The Sims 4 Seasons, Sims have the oppurtunity to outright kill an elder sim by gifting them a flower bouquet, scented with the death flower. The death animation remains the same as if the elder died by overexertion.
  • The DLC's include more "death types" like "Death by Lightning" (Seasons) or "Death by Poison" (Jungle Adventure).
  • In the "tiny living" expansion pack, the sim can sometimes be crushed by the murphy bed.
  • The Sims 4 includes 11 different death types: Drowning, Electrocution, Hunger, Fire, Rocket Ship Crash Landing, Old Age, Overexertion, Cowplant, Embarrassment, Anger, and Hysteria.
  • While not graphic or explicit, The Sims 4 Get to Work is scheduled to include the detective career and this also includes murder cases. However, blood or any violence details is yet to seen.

Profanity

  • Although players can not understand what they are saying sims can argue, yell, and insult eachother in dialogue.
  • The Sims 4 Parenthood introduces an interaction to 'Shout Forbidden Words', which is the equivalent to swearing. Doing this will lower the 'Manners' character value, and sims with the Tense or Angry Emotion are more likely to use this interaction autonomously.

Alcohol, Drugs & Smoking

  • With The Sims 4 Get Famous, celebrities consuming enough bar beverages (bar Water, Mineral Water, Creme-Cola and other non-alcoholic beverages) may gain the "Juice Enthusiast" quirk, provided a free quirk slot has been unlocked. This quirk, being similar to the alcohol addiction, presses the sim to continue consuming bar beverages at a regular interval, providing a negative buff if not remedied. This quirk cannot be treated in normal circumstances.
  • With the Sims 4 City Living, sims may smoke the water pipe ingame, which emits water bubbles. Improper use of the water pipe however makes the sim dazed.

  • With the Sims 4 Eco Lifestyle, sims may produce (partially alcoholic) beverages from the "Juice Fizzing" Machine, which includes Fizzy Juice, Seltzer, Kombucha and Honey Mead. Drinking too much Kombucha results in the sim getting dazed.
  • The Sims 4: Discover University reintroduces Juice Pong (the Sims-equivalent of Beer Pong), and Juice Kegs, which allows the sims to do a keg stand (Sims with the Bro Trait are more likely to succeed)
  • Sims can purchase a home bar and can learn the mixology skill.
  • Though the bottles are not specifically detailed, it can be assumed that they are alcoholic.
  • Sims can get dazed (which is the equivalent to drunk) by too much drinks and coffee.

Frightening & Intense Scenes

  • When a sim dies the grim reaper shows up and slashes the dead sim with a scythe who then disapears. Sims who witness this are seen crying. A pot appears resembling the dead sim. (In which afterwards the player can select "release to netherworld" making it disappear, or bring the sim back as a ghost). This may be a little intense to some players at least for the first time the player plays through this.
  • House fires may be intense for young players. Sims are seen panicking and have to extinguish the fire. Sims also can catch on fire which can kill the sim if not put out.
  • The grim reaper and sims that are ghosts (as well as aliens or vampires from expansion packs) will make sims react frightened, for example sims can be haunted by ghosts or abducted by a ufo. However these should not be seen as frightening to the player even to young viewers.
  • With the November 13, 2018, patch, most of the stronger content has been amplified due to being viewable in the first-person perspective.
  • The Sims 4 is more light-hearted compared to its predecessors, so the most frightening scenes are rather mild.

Violence & Gore

  • The Sims 4: Get Famous features stronger (but still frequently stylized) violence involving weapon fights and minor injury details, and, with the patch from November 13, 2018, introduces a first-person camera, allowing the player to view the scenes from their perspective.
  • Also in The Sims 4: StrangerVille, when being employed in the Military career, the Sims are given the choice to spar with any acquainted Sim aged Teen or older. While being a friendly sparring fight, martial arts violence can be seen, however, it will not result in injury.

Frightening & Intense Scenes

  • In The Sims 4: StrangerVille, Sims who have been affected by Spores or have eaten a Bizarre Fruit may become Infected, which causes them to occasionally lose control and become "Possessed". These Sims have an exaggerated expression and only retain a few interactions special to them. Seeing the expressions of the "possessed" Sims may be disturbing to younger viewers.
